Arizona Tribune - Menorah Life - Notice of Data Security Incident

What Happened

On September 19, 2024, Menorah Life was notified of a data security incident by its third-party vendor, PointClickCare. PointClickCare stated it became aware of a security event on July 20, 2024, that involved unauthorized access of certain end-user credentials, including one of Menorah Life's accounts. PointClickCare's investigation determined that the protected health information of some of Menorah Life's patients was accessed or acquired by an unauthorized actor between June 27, 2024 - July 22, 2024.

What Information Was Involved

The information involved varied by individual but may have included individuals' names, Social Security numbers, Medicaid and Medicare IDs, treatment, prescription, and diagnosis information, admission and discharge dates, and health insurance policy numbers.

Patients whose information was involved were notified on January 29, 2025.

What is Menorah Life Doing?

Menorah Life takes this incident seriously and has implemented additional measures to prevent future occurrences. Menorah Life has reported the incident to the Department of Health & Human Services Office of Civil Rights. Additionally, Menorah Life is offering complimentary credit monitoring and identity protection services, free of charge, to impacted individuals through Cyberscout, a TransUnion company. What Impacted Individuals Can Do

As a general matter, one should remain vigilant by reviewing credit reports, financial account statements, and explanation of benefits forms for suspicious activity and to detect errors. We also remind everyone that individuals are entitled to one free credit report annually from the each of the three major credit reporting bureaus: TransUnion, Experian, and Equifax. Individuals can further educate themselves regarding identity theft, fraud alerts, credit freezes, and the steps to take to protect personal information by contacting the credit reporting bureaus, the Federal Trade Commission (FTC) or state Attorneys General. The FTC also encourages those who discover that their information has been misused to file a complaint with them.
